By accessing or using the Care Align platform ("Service"), you agree to be bound by these Terms of Service. If you do not agree to these terms, do not use the Service.

Care Align is a provider discovery, matching, and referral platform. The Service helps users identify healthcare providers based on non-clinical preferences such as location, specialty, insurance, and care style. Care Align is not a healthcare provider, does not provide medical services, and does not replace the professional judgment of licensed healthcare professionals.

Care Align does not:

• Provide medical advice, diagnoses, or treatment recommendations
• Host or manage appointment booking systems
• Guarantee provider availability, qualifications, or outcomes
• Verify insurance coverage or benefits
• Act as an intermediary in the patient-provider relationship

By using Care Align, you acknowledge and agree that:

• You are responsible for independently confirming provider availability, credentials, and insurance participation before scheduling an appointment.
• You are responsible for verifying that your insurance plan covers services provided by any matched provider.
• You are responsible for confirming all provider details directly with the provider's office prior to any appointment.
• Provider profiles on Care Align are submitted by providers themselves and are not independently verified in real time. Care Align performs a review process but does not guarantee accuracy.
• You will not use the platform for any unlawful purpose or in violation of any applicable laws or regulations.

Care Align does not guarantee provider availability, appointment scheduling, treatment outcomes, or insurance coverage. Match results are based on stated preferences and available provider data at the time of the search. Providers may have changed their availability, insurance participation, or practice details since their profile was last updated.

Care Align is not responsible for any harm, loss, or dissatisfaction arising from interactions with providers found through the platform.

Care Align does not replace professional medical judgment. The platform is a discovery tool to help you find providers — it is not a substitute for a consultation with a licensed healthcare professional. All medical decisions should be made in consultation with qualified healthcare providers.

If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use Care Align in an emergency situation.

Providers who register on Care Align represent that the information they submit is accurate, current, and complete. Providers are responsible for maintaining the accuracy of their profiles. Care Align reserves the right to remove or suspend provider profiles that contain inaccurate, misleading, or inappropriate information.

Provider profiles are subject to an admin review process before appearing publicly. Approval does not constitute endorsement of the provider by Care Align.

All content, design, and functionality of the Care Align platform is the property of Care Align and is protected by applicable intellectual property laws. You may not reproduce, distribute, or create derivative works without express written permission.

To the maximum extent permitted by law, Care Align and its operators shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of or inability to use the Service, including but not limited to damages for personal injury, medical outcomes, loss of data, or loss of revenue.

We reserve the right to modify these Terms at any time. Changes will be effective upon posting to the platform. Continued use of the Service after changes constitutes acceptance of the revised Terms.

These Terms are governed by and construed in accordance with applicable law. Any disputes arising from these Terms or your use of the Service shall be resolved through good-faith negotiation or, if necessary, binding arbitration.
